All opinions and comments on our web sites are based on our own personal opinions and experience and/or from comments by our patients.  You should seek the advice of a professional regarding your particular health or medical situation and for a specific diagnoses, treatment, or therapy for your own individual needs.  Primary and Secondary and even Tertiary consultations with other providers/clinics are encouraged before you make any treatment decisions.  Some treatments may only produce temporary results and may need regular maintenance treatments.  Many treatments can provide long term benefits.  Some patients simply will not respond to a treatment and the result may be minimal or not help at all.  There is no guarantee when doing medical or surgical or cosmetic procedures.  No one can guarantee a result.

ALL SERVICES AT OUR OFFICE ARE NON-REFUNDABLE.  SERVICES ARE GENERALLY CLASSIFIED AS COSMETIC AND OR NEW TECHNOLOGY AND ARE NOT USUALLY REIMBURSIBLE BY INSURANCE.  OUR OFFICE  WILL NOT PARTICIPATE IN ANY INSURANCE OR THIRD PARTY PAYOR CLAIMS AND WILL NOT BE INVOLVED IN FILING INSURANCE CLAIMS, MEDICAL SAVING PLANS, GOVERNMENT OR DISABILITY CLAIMS OR ANY OTHER PAPERWORK.

THIS OFFICE OPERATES AS A LASER TYPE FACILITY AND DOES NOT TAKE ON THE INDIVIDUAL OR DAILY MEDICAL CARE OF ANY INDIVIDUAL.  ALL CUSTOMERS ARE EXPECTED TO HAVE A PRIMARY MEDICAL PHYSICIAN DIRECTING AND RESPONSIBLE FOR THEIR DAILY CARE AND SHOULD FOLLOW-UP WITH THIS PRIMARY CARE DOCTOR FOR ANY PROBLEMS, ISSUES, OR MEDICAL EMERGENCIES.  IF YOU ARE HAVING AN EMERGENCY AND YOUR PRIMARY DOCTOR IS NOT AVAILABLE YOU ARE RESPONSIBLE FOR YOUR CARE AND IT IS OUR SUGGESTION THAT YOU CALL 911 OR GO TO A LOCAL EMERGENCY ROOM.  ALL COSTS ARE THE RESPONSIBILITY OF THE CUSTOMER/PATIENT/CLIENT.

OUR OFFICE DOES NOT KEEP OR PARTICIPATE IN ELECTRONIC RECORDS AND IS NOT SUBJECT TO THE HIPAA LAWS WHICH WERE PASSED FOR ELECTRONIC RECORDS PROVIDERS.  THUS, DO NOT CONFUSE ACCESS LAWS FOR PATIENT RECORDS WRITTEN SPECIFICALLY FOR ELECTRONIC MEDICAL RECORDS/ELECTRONIC INSURANCE TRANSACTIONS HEALTH CARE PROVIDERS.  OUR OFFICE DOES NOT PARTICIPATE WITH ANY INSURANCE OR THIRD PARTY PAYORS.  THERE ARE NO ELECTRONIC RECORD EXCHANGES. OUR OFFICE IS NOT A COVERED ENTITY UNDER HIPAA.

OUR OFFICE ALSO CHARGES COPYING, MAILING, INSPECTION AND ADMINISTRATIVE FEES IF RECORDS ARE SENT OUT.  FOR NON-ELECTRONIC RECORDS THE MEDICAL PAPER RECORD IS THE PROPERTY OF THE BUSINESS BY A LONG HISTORY OF COURT RULINGS.
OUR ONLY OBLIGATION FOR NON-ELECTRONIC RECORDS IS TO A COURT OF LAW.  WE MAY PROVIDE WITH PROPER CONSENTS AND RELEASES A COPY OR SUMMARY OF A TREATMENT TO ANOTHER TREATING HEALTH PROVIDER.

IF A PATIENT OR PARTY WISHES TO WRITE A CHECK AT OUR OFFICE FOR PAYMENT THEN WE MAY ASK FOR A SOCIAL SECURITY NUMBER OR DRIVERS LICENSE NUMBER BECAUSE OFTEN LOCAL DISTRICT ATTORNEYS OR COLLECTION AGENCIES REQUIRE THESE TO FOLLOW UP ON BAD CHECK PAYMENTS FOR COLLECTIONS AND CRIMINAL PROSECUTIONS.

OUR OFFICE RESERVES THE RIGHT TO REFUSE TREATMENT
OR A SERVICE TO ANY INDIVIDUAL OR PERSON.
